	Full Day (6 HOURS) for 2-3 anglers $350.00 total with balance due at end of trip
	Full Day (6 HOURS) for 4 anglers $400.00 total with balance due at end of trip
	3 Hour Evening Detroit River Must be paid in full- $50/angler with a $100 minimum
	Saturday or Sunday Detroit River(6 HOURS) for 1-4 anglers $400 total with balance due at end of trip

****Cancellations based on weather/water conditions are the final decision of the Captain. In the event of cancellation by the Captain the trip will be rescheduled. Captain may end the trip early for any unsafe or lewd behavior by any of the guests.

****NO ALCOHOL OR NON PRESCRIPTION DRUGS ALLOWED ON BOARD AT ANY TIME.

****Cancellations by the client less than 15 days prior to the trip will forfeit full amount of deposit. Cancellations 16-30 days prior will forfeit 1/2 of deposit. All other cancellations can be applied to another date.

****By signing this form you agree to hold harmless “Lance Valentine’s Walleye 101 Inc.” and any and all promoters, agents, agencies, employees and organizations assisting or providing services for the operation of the trip. Furthermore, you agree to waive all responsibility for death, injuries, damages, liability, theft, fire or loss to person and/or equipment of any kind. No unsigned entry will be accepted. All anglers must sign this form before boat leaves harbor.
